Birth Certificates are issued at the Driver License Office ONLY

In partnership with the Florida Department of Health’s Bureau of Vital Statistics, certified copies of Florida  Birth Certificates can be obtained from the Driver's License office of the Tax Collector's Office 

Only simple applications can be processed. Applications are only accepted from either the child named on the birth record (if 18 or older) or a parent named on the birth record. Valid identification must be presented at the time of application. 

Primary types of valid Photo IDs: 

• Driver’s License

• State ID Card, issued by the Department of Motor Vehicles

• Passport

• Military ID

Each copy of a Birth Certificate printed is $21.25. Acceptable forms of payment: Cash, checks, money order, or credit/debit card (2.5% convenience fee applies).
